RN Case Manager – Home Health

Organization: Adoration Home Health | Monument

Location: Owensboro, KY

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

On-Call: Every 3rd or 4th week/weekend (required)

Hours: 40 hours/week

Contract Length: 13 weeks

About the Role

Adoration Home Health is seeking an experienced RN Case Manager to support patients across the Owensboro area. This role is ideal for nurses who enjoy autonomy, coordination of care, and making a direct impact on patient outcomes in the home health setting.

The RN Case Manager is responsible for coordinating the continuum of care for assigned patients while ensuring efficient use of resources, compliance with care plans, and high-quality service delivery.

Key Responsibilities

  • Perform comprehensive assessments of high-risk patients

  • Develop and manage individualized, patient-centered care plans

  • Collaborate closely with physicians, primary care providers, social workers, and discharge planners

  • Coordinate transitions of care and appropriate levels of service

  • Identify and address non-medical barriers to care such as transportation or housing

  • Engage specialty resources as needed to support care goals

  • Monitor clinical, functional, and fiscal outcomes throughout the case-management process

  • Promote preventative care and health education

  • Advocate for patient privacy, safety, and quality outcomes

  • Maintain accurate and timely documentation

Coverage Area & Productivity

  • Counties served: Daviess, McLean, Ohio, Webster, Henderson

  • Productivity expectation: 30 points per week (6 points per day)

  • Mileage reimbursement: $0.43 per mile

Minimum Qualifications

  • Active Kentucky RN license

  • At least one year of case-management experience preferred

  • Home health experience strongly preferred

  • Proficiency in skilled nursing care (IV therapy, wound care, wound vacs, etc.)

  • CPR certification

  • TB/health clearance per state requirements

  • Valid driver’s license and auto insurance

  • Ability to analyze complex medical information and make sound clinical decisions

  • HCHB experience preferred

Compensation

  • Local Pay Rate: $52.05/hour

  • Weekly pay

  • Overtime paid at 1.5x
