Provide personalized, hands-on care to hospice patients and support their families—wherever they call home.
Qualifications:
Active NJ CHHA certification (required)
Compassion for end-of-life care
Strong communication and teamwork skills
Reliable transportation
Responsibilities:
Provide personal care and assist with daily living activities
Support patients with dignity, comfort, and compassion
Observe and report changes in patient condition
Collaborate with the hospice care team

About You
Qualifications – What You’ll Bring:
Active CHHA license in the state of employment (Required)
Current CPR certification (Required)
Driver’s license and reliable vehicle with insurance (Required)
Six (6) months of direct patient care experience (Required)
Comfortable working in home settings, hospice facilities, or long-term care environments
Demonstrated compassion, patience, and ability to handle emotional situations
Willingness to work independently and adapt to changing patient needs
Preferred Background (Not Required):
Experience in home health, hospice, palliative care, or oncology
Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
Exposure to end-of-life care, wound care, or admissions support
