Licensed Practical Nurse - Norfolk, Virginia

Job Description

Coordinate and provide Medicare Wellness Visits to eligible patients in the primary care practice. Work closely with medical assistants and other team members to ensure comprehensive care for patients identified with quality gaps or in need of Medicare Wellness Visits.

Responsibilities

  • Utilize Excel spreadsheets to track patients due for Medicare Wellness Visits and schedule appointments.

  • Collaborate with medical assistants and practice team members to review patient lists, quality measures, and outreach lists.

  • Attend Care Management Meetings, Primary Care Collaborative meetings, and educational sessions as scheduled.

  • Follow chronic disease and preventive care guidelines, developing appropriate plans for completion.

  • Assist patients in developing personalized wellness plans with realistic goals, including advanced care planning.

  • Educate and support patients in making choices that lead to optimal well-being and higher quality of life.

  • Promote patient engagement and compliance with appointments, treatment recommendations, and other services.

  • Complete all tracking and reporting requirements for outcomes and evaluation.

  • Perform phone triage, patient scheduling, call pharmacies, authorize refills, and respond to tasks and patient messages.

  • Prepare exam rooms, restock supplies, sterilize instruments, and maintain medical equipment.

  • Maintain knowledge of office procedures such as ECG, in-house labs, spirometry, nebulizer treatments, and minor surgical assistance.

  • Document all nursing interventions in the electronic medical record, ensuring completeness and accuracy.

  • Respond to questions from patients, family members, faculty/residents, and other healthcare providers promptly and professionally.

  • Follow infection control, environmental safety, and patient confidentiality protocols.

  • Complete annual recertifications for OSHA, compliance, respiratory FIT, and IT policy.

  • Maintain current CPR certification and nursing license.

Essential Skills

  • Licensed LPN in Virginia.

  • One year of LPN experience preferred.

  • Completion of BLS class and re-certification every two years.

  • Ability to make professional nursing decisions within the scope of practice for licensure.

  • Complex patient assessment and management skills for developing care plans for Medicare Wellness Visits.

  • Basic knowledge of educational behavioral approaches to change behavior.

  • Professional interaction skills with physicians, patients, and healthcare personnel.

Additional Skills & Qualifications

  • Care management and coordination skills.

  • Experience with ambulatory care and case management.

  • Familiarity with electronic health records, especially Epic.

  • Triage and managed care experience.

  • Ability to develop care plans including medication lists, individualized patient goals, and review of chronic diseases.

Work Environment

The shift is Monday to Friday, 8 AM to 5 PM, with an hour lunch break. The facility is a learning environment offering opportunities for growth and exposure to a variety of illnesses and treatments.

Job Type & Location

This is a Contract to Hire position based out of Norfolk, VA.

Pay and Benefits

The pay range for this position is $27.00 - $30.00/hr.
