Norton Sound Health Corporation Care Coordinator Assistant in Nome, Alaska
Purpose of Position:
Assist Care Coordinators in the assigned departments through scheduling, reception, record, maintenance and coordination with outpatient clinic and specialty clinic personnel in the management of patient care.
Uphold the organizations vision, mission, and corporate values. Demonstrate understanding of and compliance with organizations policies, procedures, code of conduct and work rules.
Essential Functions:
- Assist Care Coordinators, nursing staff and medical providers in maintaining a variety of files, lab order entry, radiology order entry, obtaining medical reports from other facilities and providers and coordinate patient appointments.
- Verify that records of patient treatment are complete, accurate and properly entered before forwarding to the Health Information Management Department.
- Under the direction of the care coordinators, assist with routine referrals in conjunction with NSHC and other providers, schedule diagnostic studies and help with travel and housing arrangements.
- Communicate with other departments, patients and family to coordinate care
- Maintain the database of clients requiring follow-up and assist with scheduling and communication with patient for the follow-up.
- Verify Medicaid enrollment for as required
- Perform other duties as assigned
