Licensed Practical Nurse (LPN)
Per Diem
Pay: $33-$52/hour based on experience and shift differentials

What you'll do as a Licensed Practical Nurse (LPN) -
Perform assessments and collect resident data to support care planning
Provide and receive shift reports to ensure continuity of care
Administer and document medications, treatments, and enteral nutrition per physician orders; follow the 6 rights of medication administration
Complete documentation, including nurses' notes, MDS/RAPs, and incident/accident reports
Coordinate resident admissions, transfers, and discharges, including required documentation
Collaborate with residents, care teams, and responsible parties to support overall health and well-being
Licensed Practical Nurse (LPN) Requirements -
High School diploma or GED
Current New Jersey LPN license
IV & CPR certification preferred (IV certification required within 6 months of hire)
Prior nursing experience in a related field preferred
