Position Summary The Department of Medicine, Division of Pulmonary, Allergy & Critical Care Medicine seeks a Patient Referral Coordinator to support patient referral processing via the Epic work queues in accordance to departmental standards. Responsibilities Monitor referral work queue and ensure information on incoming referrals is accurate and that the patient?s symptoms appropriately reflect the requested treatment. Liaise with referring providers to inform and encourage patients to schedule referrals, introduce your providers, and educate them of your services. Assist with referral process to ensure equitable distribution among providers and reduce referral errors. Prioritize referrals based on clinical priority and provider availability. Work with designated Attending/RN screener for undifferentiated diagnosed patient referrals. Coordinate and provide support for all referral appointments as established by practice protocols. Maintain current knowledge of insurer referral/authorization requirements. Have a working understanding of the patient population and demonstrate cultural awareness and sensitivity. Consistently monitor clinic schedules to ensure maximized scheduling. Fills available/open timeslot including those appointments canceled and/or rescheduled. Ensure the patient referral is properly linked to the appointment. Update the referral scheduling record status with each new development. Perform additional related duties as assigned.
