Assistant Director of Nursing (ADON)

Full-Time | Skilled Nursing / High-Acuity LTC

New York, NY

A fast-paced Skilled Nursing Facility in New York City is seeking an experienced Assistant Director of Nursing (ADON) to partner with the DON in overseeing daily clinical operations. This role is designed for a strong RN leader who thrives in high-acuity environments and is comfortable operating under the regulatory intensity of NYSDOH.

This building supports complex residents including sub-acute rehab, IV therapy, dialysis coordination, advanced wound care, and medically fragile populations. The ADON must be highly visible on the floor and operationally strong across all shifts.

Key Responsibilities

Clinical & Regulatory Leadership

  • Support oversight of all nursing services and ensure full NYSDOH/CMS compliance

  • Maintain continuous survey readiness

  • Assist with audits, investigations, and Plans of Correction

Team Leadership & Staffing

  • Support recruitment, onboarding, and retention of RNs, LPNs, and CNAs

  • Provide clinical mentorship and supervision to Unit Managers and nursing staff

  • Assist with staffing grids, overtime control, and agency management

High-Acuity Care Oversight

  • Monitor care plans and MDS accuracy

  • Provide hands-on support during high-volume or high-acuity shifts

  • Collaborate with physicians and interdisciplinary teams to reduce rehospitalizations

Quality & Performance Improvement

  • Participate in QAPI initiatives tied to 5-Star performance

  • Track and trend KPIs including falls, infection rates, wounds, and readmissions

Requirements

  • Active, unencumbered New York RN license

  • 3–5+ years Skilled Nursing/LTC experience

  • Prior leadership experience (Unit Manager, Nurse Supervisor, or ADON preferred)

  • Strong NY DOH survey and MDS knowledge

  • Experience in high-acuity Skilled Nursing settings

  • EHR experience (PointClickCare or SigmaCare preferred)

Pay Details: $130,000.00 to $140,000.00 per hour
