How you’ll make an impact in this role
Design Person-Centered Care:Build comprehensive transition plans that bridge the gap between hospital and home, ensuring every step reflects the patient's unique needs and goals.
Champion Patient Advocacy:Proactively identify and dismantle barriers to care, ensuring your patients have the resources and support systems they deserve.
Act as a Clinical Subject Matter Expert:Serve as the go-to resource for the care team on complex issues, including crisis intervention, neglect, and adjustment needs.
Drive Care Excellence:Maintain a continuous, collaborative pulse with supervisors and transition teams to refine health plans and ensure seamless implementation.
What minimum requirements you’ll need
Licensure / Certification / Registration:
One or more of the following required:
Clinical Counselor obtained prior to hire date or job transfer date.
Bachelor Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
