Integrated Community Home Care Solutions Direct Support Professional/Home Health Aide in Mt Penn, Pennsylvania
Responsibilities:
- The employee will provide direct care services in a residential setting and complete the assigned shift at the assigned site.
- Complete all shift assignments and tasks, complete all site documentation (ISP's, logs, etc.)
- Support Individuals with daily living skills in the home and community.
- Encourage independence, respect and skill development
- Transport and/or assist Individuals to and from appointments, activities, and visitations. (Company vehicle provided; requires a valid driver's license for Dayshift).
- Assist with meals, personal care and household tasks.
- Maintain a safe, supportive and inclusive environment
- Must be able to pass background check.
- No more than 3 Individuals per home in a safe, clean, healthy environment.

- Must be available every other weekend
Education:
- High school or equivalent (Preferred)
License/Certification:
- Driver's License (Preferred)
Work Schedule:
- 3 days on/4 days off one week,following week would be 4 days on/3 days off.
- 36 hours one week, 48 hours the next week.
Bilingual Pay Differential: Bilingual applicants receive a $1.00 pay differential.
