Position Summary
The Qualified Mental Health Specialist (QMHS) at TPC Wellness -- The Phoenix Center provides high-quality, client-centered behavioral health services under the supervision of a licensed clinician. The QMHS supports Individual Treatment Plan (ITP) management, Level of Care (LOC) determinations, and delivers both group and individual therapeutic services in alignment with Ohio QBHS standards.
This role emphasizes collaboration with case management, treatment planning, care coordination, and community-based supports to promote recovery, resilience, and improved functioning across life domains.
Essential Duties and Responsibilities
- Assist in development, implementation, and monitoring of Individual Treatment Plans in collaboration with clients, families (as appropriate), and the clinical team.
- Collect and document progress toward treatment goals, symptoms, strengths, and risk factors.
- Participate in case conferences and clinical reviews.
- Ensure treatment goals are evidence-based, culturally responsive, person-centered, and aligned with payer/regulatory requirements.
- Collaborate with the treatment team to conduct LOC determinations using Ohio QBHS criteria.
- Monitor client status changes and communicate recommendations for LOC adjustments.
- Facilitate transitions across levels of care (outpatient, IOP, community-based services, inpatient referrals).
- Support discharge planning and follow-up to ensure continuity of care.
- Plan, facilitate, and document psychoeducational and skills-building groups (coping skills, stress management, relapse prevention, mindfulness, social skills, DBT-informed groups).
- Create trauma-informed, culturally sensitive group environments.
- Maintain attendance records, materials, and EHR documentation in compliance with confidentiality standards.
- Provide structured, goal-oriented individual sessions.
- Utilize appropriate therapeutic modalities under supervision (psychoeducation, motivational interviewing, CBT-informed strategies).
- Respond to crisis situations per agency protocol, including safety planning and escalation when needed.
- Maintain timely and compliant clinical documentation.
- Collaborate with case managers to address barriers (housing, employment, transportation, legal, social supports).
- Assist with referrals, resource linkage, and follow-up.
- Track and report client outcomes for quality improvement.
- Adhere to Ohio QBHS standards, Medicaid/managed care requirements, and agency policies.
- Maintain confidentiality in accordance with HIPAA and 42 CFR Part 2.
- Participate in supervision, trainings, chart reviews, and quality initiatives.
- Complete all EHR documentation accurately and timely.
- Submit required reports and performance metrics.
Maintain up-to-date credentialing and licensure requirements.
Education
- High school diploma or equivalent required.
- Bachelor's degree in Psychology, Social Work, Counseling, or related field preferred.
- Associate degree with equivalent clinical experience may be considered.
- Valid driver's license and reliable transportation (if community-based services required).
Experience
- 2--3 years of experience in behavioral health preferred.
- Experience with QBHS-style care coordination, LOC management, or recovery-oriented services preferred.
- Experience delivering group and individual services preferred.
Skills
Knowledge of evidence-based practices and QBHS standards.
Strong interpersonal and motivational interviewing skills.
Crisis intervention and de-escalation abilities.
