Job Duties
We are seeking a Designated Coordinator (DC). The DC is responsible for coordinating and implementing services for individuals receiving 245D for Day Services and Employment Supports. The DC ensures services are delivered in alignment with each person’s CSSP addendum, organizational polices and 245D licensing standards. The DC serves as the primary point of contact for individuals receiving services, their support teams, and funding entities. Ensuring open channels of communication between the team and CEI. The DC works collaboratively with the program managers to ensure services are aligned with individual goals- seeking input from the person served…it’s a team!
Minimum Education Required
• 2 year post-secondary degree in human service- related field, education or health + 3 years FTE direct care services
OR
• A minimum of 50 hours of education/training related to human services and disabilities + 4 year FTE providing direct care services
Minimum Experience Required
• 1–3 years of experience in program coordination, nonprofit work, or related field
• Strong organizational and time management skills
• Excellent written and verbal communication skills
• Ability to manage multiple priorities and work independently
• Proficiency in Microsoft Office / Excel
• Familiarity with 245D standards
