Position Summary
We are seeking a skilled and dependable Wound Care Nurse (RN or LPN) to provide in-home wound care services to our patients. The ideal candidate is clinically strong, detail-oriented, and experienced in wound assessment, treatment, and documentation.
Key Responsibilities
Perform comprehensive wound assessments and measurements Provide wound care treatments per physician orders (including complex wounds) Change dressings and manage wound vacs (if applicable) Monitor for signs of infection or complications and report changes promptly Educate patients and caregivers on wound care and prevention Accurately document visits, wound progress, and patient response to treatment Coordinate care with physicians, supervisors, and interdisciplinary team Maintain compliance with state regulations, agency policies, and standards of care
Requirements
Active Virginia RN or LPN license (in good standing) Minimum 1 year of wound care experience (home health preferred) Knowledge of wound types (pressure injuries, surgical wounds, diabetic ulcers, etc.) Strong assessment and documentation skills Reliable transportation and valid drivers license Ability to work independently and manage time effectively Current CPR certification TB screening and background check required
