Job Description:
Schedule: Per Diem - Requirement of 4 shifts per month. Orientation of Mon-Friday for first 2 weeks on dayshift.
Location: Covering the Shawnee Mission Area
• Develops plans of care for psychosocial needs of patients.
• Assists patients and families with financial needs and resources.
• Provides counseling to patients and their families.
• Understands and addresses end-of-life issues.
• Works independently while maintaining case load expectations.
• Collaborates with interdisciplinary teams to implement and evaluate care plans.
• Conducts initial psychosocial evaluations and ongoing support.
• Refers patients and families to community resources as needed.
• Assists with Medicaid applications and placement in nursing homes or assisted living facilities.
• Performs other duties as assigned.
• Engages in community education and outreach activities.
• Supports patients and families in achieving desired outcomes through effective social work interventions.
• Performs other duties as assigned.

Education:
• Master's [Required]
Field of Study:
• Psychology, sociology or social work
Work Experience:
• 1+ year of experience in a healthcare setting [Required]
• 2+ years of experience in the social work field [Preferred]

Licenses and Certifications:
• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]
• Clinical Social Worker License (LCSW) [Preferred]

Pay Range:
$24.01 - $44.65
