FTE: 0.001000 Standard Hours: 0.01 Shift: 1st Job Summary: * The Health Coach will assist with the health appraisal process- accurately conducts point of care testing at the worksite which may include: cholesterol, blood pressure, heart rate and glucose screening. Completes external medical referrals based on clinical values and following departmental policies and procedures. * Provides evidence-based individual/group/ and or telephonic health coaching, utilizing motivational interviewing. * After screening, develops SMART goal in conjunction with client. Adheres to brand standard, all departmental P&P. Remains within scope while working with clients. * Accurately navigates and utilizes software systems (e.g. When to Work, PDHI, Outlook). * Tracks participation, documents completely in paper or electronic record. * Gathers confidential data, with timely and accurate documentation following HIPPA guidelines. * Assures documentation of coaching completion to track incentive administration as needed. Participates in development of projects supporting the department workflows. * Remains abreast of department process changes and educational changes through attendance at staff meetings and other departmental communications. The Health Coach will assist with assessing, planning, designing, and implementing the Wellness Works or Workforce Health program, as well as presenting on various health topics to corporate clients. This individual will assist and interact with departments to develop services that meet the needs of Froedtert Health customers. Travel is required. Other duties as assigned. EXPERIENCE: * Two to three years of corporate wellness/health education/program development/community relations/presentation experience is required. * Healthcare organization experience is preferred. * Knowledge of Motivational Interviewing strategies preferred. EDUCATION: * Bachelor\'s degree in health/community education, exercise physiology, or related healthcare/wellness field is required. * A Master\'s degree with an emphasis in the health care field is preferred. LICENSURE: * Valid Driver\'s License is required. * American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within three months of hire. * Wellness or Health Coaching certification endorsed by NSHC, ACE or Wellcoaches is preferred. SPECIAL SKILLS: Working knowledge of personal computer and various audiovisual equipment including Microsoft Suite. Demonstrates evidence of group facilitation, integrity, initiative, written and verbal communication skills.
