Home Health Occupational Therapist
Full Time
Coverage area: Newaygo, Mecosta, Mason, Oceana Counties

What You’ll Do:
Evaluate patients and develop individualized plans of care based on functional needs and goals
Perform initial and ongoing assessments in accordance with OASIS and regulatory guidelines
Deliver high-quality occupational therapy services in the home setting
Educate patients and caregivers on ADLs, adaptive equipment, and home safety
Collaborate with physicians and interdisciplinary team members to ensure continuity of care
Adjust care plans as needed and obtain physician orders for changes
Support discharge planning and long-term patient success
Provide guidance and supervision to COTAs and other clinical team members

What is Required?
Graduate of an accredited Occupational Therapy program
Current, unrestricted OT license valid for the state of work
Minimum one (1) year of experience in a clinical care setting
Able and willing to travel within branch/office coverage area.
Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Must be able and willing to travel 50%
Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
