Full-time RN position to provide individualized nursing care at the place of residence/home of clients with acute, chronic and complex conditions.
- Home Care Registered Nurse (RN) – MercyOne/Trinity Health Home Care, Mason City
Monday - Friday 8am-4:30 pm; Weekend rotation based on how many nurses are employed. On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.

MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. In this rewarding role, you’ll deliver high-quality, personalized care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life.
What You’ll Do
Perform comprehensive patient assessments and identify care needs.
Develop and implement individualized care plans in collaboration with interdisciplinary teams.
Administer medications and treatments, monitor patient responses, and adjust care as needed.
Educate patients and families on disease management, medications, and wellness strategies.
Monitor vital signs and manage chronic conditions with timely interventions.
Accurately document all care activities and maintain compliance with healthcare regulations.
Coordinate care with physicians, therapists, and other healthcare professionals.
Provide emotional support and guidance to patients and their families.
Participate in ongoing education and quality improvement initiatives.
What You’ll Need
- Minimum Qualifications: • Graduate of an approved Nursing education program, BSN preferred.• Current Registered Nurse licensure in the state of practice.• Must have a minimum of one (1) year professional nursing experience.• Home healthcare experience preferred.• Strong computer and technology skills required. • Must have current Driver’s license and reliable transportation.• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
