Full-time RN position to provide individualized nursing care at the place of residence/home of clients with acute, chronic and complex conditions.

- Home Care Registered Nurse (RN) – MercyOne/Trinity Health Home Care, Mason City

Monday - Friday 8am-4:30 pm; Weekend rotation based on how many nurses are employed. On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.

MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. In this rewarding role, you’ll deliver high-quality, personalized care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life.

What You’ll Do

  • Perform comprehensive patient assessments and identify care needs.

  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.

  • Administer medications and treatments, monitor patient responses, and adjust care as needed.

  • Educate patients and families on disease management, medications, and wellness strategies.

  • Monitor vital signs and manage chronic conditions with timely interventions.

  • Accurately document all care activities and maintain compliance with healthcare regulations.

  • Coordinate care with physicians, therapists, and other healthcare professionals.

  • Provide emotional support and guidance to patients and their families.

  • Participate in ongoing education and quality improvement initiatives.

What You’ll Need

  • Minimum Qualifications: • Graduate of an approved Nursing education program, BSN preferred.• Current Registered Nurse licensure in the state of practice.• Must have a minimum of one (1) year professional nursing experience.• Home healthcare experience preferred.• Strong computer and technology skills required. • Must have current Driver’s license and reliable transportation.• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
