Southwestern Center for Independent Living CDCS Program Manager in Marshall, Minnesota
The Southwestern Center for Independent Living (SWCIL) is an independent living resource center that works to promote societal acceptance, inclusion, and equal access for all persons with disabilities living in Southwest Minnesota. A Consumer Directed Community Supports (CDCS) Program Manager oversees and provides services to individuals in the development of Consumer Directed Community Supports (CDCS) plans, budgets, and amendments; ensures the delivery of high-quality program services; and implements program updates in accordance with applicable requirements. Candidates must reside in the local Southwestern Minnesota service area including Cottonwood, Jackson, Lincoln, Lyon, Murray, Nobles, Pipestone, Redwood, Rock, and Yellow Medicine Counties. Responsibilities include: Is familiar with the Consumer Directed Community Supports (CDCS) service, person-centered planning, and the process of developing a support plan. Develops individualized support plans tailored to the unique needs of each individual. Maintains accurate documentation and data collection related to consumer progress and service delivery. Ensures quality and updated program services. Provides outreach in the local service area. Is familiar with community resources and support services. Some local travel required.
