JOB REQUIREMENTS: The Children\'s Long Term Wavier Specialist is an important part of the Family Resiliency Unit of Manitowoc County Human Services Department. The Children\'s Long-Term Support (CLTS) Program helps children with disabilities and their families through supports and services that help children grow. This position authorizes and coordinates services to assist these families to care for their children in their home and will: Provide case management through contacts with families and service providers. Participates in collaborative meetings and attends appointments Submit timely and accurate documentation for each case file and maintain case records. Process referral forms, interviews collateral sources, and administers functional screens to determine if the individual is eligible for services and the level of care and services needed.
