Job Summary and Qualifications
The Manager of Case Management supports the Director of Case Management in managing Case Management operations. This includes providing leadership for planning, management, implementation, integration, and coordination of Case Management in support of facility goals dedicated to excellence, effective leadership, and financial stability. The role supports the delivery of high-quality, cost-effective patient care through activities based on facility strategic goals and objectives.
Assist in the planning and coordination of Case Management activities.
Provide direction to the Case Management team and assist in performance reviews.
Ensure adherence to regulatory requirements.
Make decisions in the Director's absence to maintain operations.
Collaborate with other department leaders to resolve procedural difficulties.
Monitor staff caseload and reorganize the department as needed.
Approve staffing schedules.
-Monitor daily reports of observation patients.
- Develop and monitor strategies for resource allocation.
-Assist team members in problem-solving complex patient situations.
Assist the Director in planning and coordinating Case Management activities, operational efficiency, and effectiveness.
Assist the Director in providing direction to the staff of the Case Management team and assists in reviewing and evaluating performance of the department staff. Involved in counseling and appropriate disciplinary actions of staff members as requested.
-Assist department staff in meeting and adhering to all applicable federal, state, JCAHO, and local regulatory agency requirements and assist in the success of the department and facilities in any regulatory survey.
Make decisions in the absence of the director to maintain acceptable operations based on strategic goals and policies. Displays ingenuity and foresight in determining the most appropriate solution in the absence of established guidelines.
Confer with other department leaders when necessary to resolve procedural difficulties, clarifying department responsibilities, objectives, and resolving identified problems.
Adhere to hospital/departmental attendance policy, work hours, and maintenance of personal appearance as stated in facility policy.
Make suggestions on the development of new system processes as needed to support new regulations, software installation, changing environment, and needs of the team.
Work with the Case Management Director to monitor staff caseload, reorganizing the department as needed to support the patient population on each unit.
Approve staffing schedules ensuring appropriate coverage to support patient case management needs.
-Monitor daily reports of observation patients. Understands financial implications of Length of Stay requirements for the organization and serves to improve efficiency while ensuring safe, appropriate discharge planning throughout the organization.
Collaboratively work with the Case Management team and effectively engage employees, maintain positive teamwork relationships, and strive to improve employee engagement.
Develop, coordinate, and monitor strategies for obtaining, organizing, and allocating resources required for care transitions.
-Assist team members in problem-solving complex patient situations.
-Establish collaborative practice with nursing personnel, physicians, and health care professionals regarding the plan of care and identify variance in plan implementation.
-Identify necessary resources or eliminate unnecessary resources needed to achieve quality patient outcomes as well as advocate for patients facing barriers to resources. This includes identifying alternative options if needed resources are not accessible.
Manage referrals for post-acute services based on Federal and State regulations, client eligibility for service, the patient’s home care needs, and the ability of the agency to safely provide the needed services.
Collaborate with the healthcare team, develop treatment goals, and plan interventions that assist the patient and family to resolve social and psychosocial concerns that may arise as a result of illness, engagement in high-risk social behaviors, limited finances, and lack of family support and/or physical disabilities.
Collaborate with post-acute discharge providers in the development of transition plans. Obtain physician involvement and agreement with the plan. Advocate for patient choice.
Provide leadership for multidisciplinary team efforts during interdisciplinary rounds (IDR’s).
Work collaboratively with peers to achieve department goals and daily work as evidenced by appropriate and timely communication, which is respectful and clear. Sensitive to the workload of the team, shares responsibilities, fills in, and offers help.
Serve as a preceptor for new hires, facilitate education and growth within the staff, and actively participate in coaching and evaluation of assigned team members.
Actively participate in conferences, committees, and task forces as directed.

What qualifications you will need:
Bachelor's degree, required
3+years experience , preferred
State of New Hampshire RN License
