Job Description:
Schedules follow-up appointments with physicians for patients discharged home.
Coordinates appointments for patients without a documented primary care physician.
Arranges specialist appointments as prescribed by the physician.
Contributes to the daily processes involved in discharging patients.
Manages patient care coordination to ensure timely follow-up and continuity of care.
Communicates with patients to confirm and remind them of their appointments.
Collaborates with healthcare providers to facilitate patient referrals and consultations.
Maintains accurate records of patient appointments and follow-up care.
Provides support to patients in navigating the healthcare system.
Ensures compliance with hospital policies and procedures in patient care coordination.
Addresses any issues or concerns related to patient appointments and follow-up care.
Performs other duties as assigned.

- High School Grad or Equiv Required
Work Experience:
3+ of hospital Preferred
