The Pharmacy Supervisor will be responsible for leading and supervising the daily functions of medication reconciliation services, programs and personnel throughout UnityPoint Health- Meriter (UPH-M). This position will facilitate the development, implementation and monitoring of medication reconciliation initiatives and provide direction and leadership to ensure UPH-M delivers high quality patient care and improved population health while improving the work life of staff. Principal responsibilities include the supervision of medication reconciliation workflows and services, management of staff, project management, quality improvement, maintaining compliance with policies, procedures and accreditation requirements, medication safety, teaching, and scholarship in associated areas of responsibility.

Education:
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
Experience:
Three years of relevant work experience, or PGY1 plus one year of relevant work experience, or PGY2
License/Certification:
Licensed as a pharmacist in Wisconsin or eligible (license in another state)
