Position Purpose
The Scheduler plays a vital role in supporting timely, compassionate Home Health care across Southeast Michigan. In this position, you will coordinate patient schedules with a focus on clinician continuity, ensuring that each individual receives care from the most appropriately skilled team members. You will use sound judgment to manage scheduling and rescheduling needs, communicate proactively with patients and care teams, and support smooth daily operations across multiple disciplines and locations. Your work helps ensure patients receive the right care at the right time—aligned with our mission of serving with dignity and respect.
What You Will Do
Schedule Home Health visits by matching client needs with clinicians who have the appropriate competencies.
Follow established policies and procedures to ensure scheduling accuracy, efficiency, and continuity of care.
Develop and maintain daily schedules for all clinical disciplines, striving to limit clinician variation within each patient episode.
Conduct initial Welcome calls to confirm client details and schedule admission visits within 48 hours.
Communicate promptly with clients regarding delays in physician‑ordered services.
Assign initial and ongoing visits, support clinician workflow tasks, and prioritize end‑of‑day completion of all assignments.
Collaborate with clinical managers to secure coverage for illnesses, vacations, and unplanned staff absences.
Review daily scheduling reports and take appropriate follow‑up actions.
Maintain accurate documentation in accordance with TIOC Standards.
Provide weekend and holiday coverage on a rotating basis and support national scheduling initiatives.
Respond quickly and professionally to staff requests while upholding Trinity Health’s Integrity Program, Code of Ethics, and regulatory requirements.
Perform additional duties as assigned by the Scheduling Supervisor or Manager.
Minimum Qualifications
High School Diploma or GED required; Associate’s degree preferred.
Healthcare experience preferred; 1–2 years scheduling in a healthcare setting highly preferred.
Understanding of medical terminology.
Strong communication skills with internal and external customers.
Ability to type 50 wpm; proficiency in Microsoft Outlook, Excel, and Word.
Working knowledge of standard office equipment.
