The role is key in educating stakeholders about home infusion offerings and ensuring compliance with industry standards, ultimately contributing to the well-being and satisfaction of patients receiving care by home infusion.Job Description:Baptist Health is looking for a Home Infusion RN Hospital Liaison to join our Home Infusion team at Baptist Health Lexington!Monday -Friday from 9am -5:30pm.On call rotating weekends for new referrals leaving the hospital.- Advocate for patients and families, ensuring their home infusion needs are properly identified andmet.- Educate patients and their families on infusion procedures, medication administration, and howto manage potential side effects.- Provide guidance on maintaining a sterile environment and proper use of infusion equipment.- Educate patients and their families on managing illnesses or injuries, including post-dischargecare and chronic condition management.- Maintain accurate, timely, and detailed documentation of all patient care activities in compliancewith department and regulatory standards.- Regular and reliable attendance is an essential function of this role.
