SummaryJob Description:The Diabetes Education Department provides patient-centered education and support to help individuals manage diabetes and related conditions. The department collaborates with interdisciplinary care teams to deliver evidence-based education, promote self-management skills, and improve health outcomes through prevention, treatment, and ongoing disease management.DietitianFull-Time, DaysJob Description:- Under limited supervision, provides professional medical nutrition therapy to groups and individuals of all ages andabilities, in a variety of settings in the outpatient nutrition service area.- Follows established standards of practiceand guidelines defined by the Academy of Nutrition and Dietetics, USDA, American Diabetes Association, and otherevidenced based resources for the assessment of patients, and subsequent planning, implementation, evaluation ofnutrition education.- Services also includes continuum of care for out-patients requiring medical nutrition therapy.- Works with an established multidisciplinary team of professionals to provide quality nutrition education servicesincluding all department services.- Also provides support the outpatient diabetes education services as needed.Job Requirements:- Bachelor's Degree- One year experience- Registered Dietitian, Commission on Dietetic Registration- Licensed Dietitian, State of Kentucky- Experience with patient education and counseling- Current registration or registration-eligible with the Commission on Dietetic Registration. Those registration-eligible mustbecome registered within 4 months of date of hire unless extension granted.- Licensed Dietitian in Kentucky or licensed-eligible after receiving CDR registration.
