Job Title: Independent Support Coordinator (ISC)

Caseload Area/Job Opening for: South Central Middle TN (Murfreesboro, Lawrenceburg, Hohenwald, Columbia areas) and Nashville surrounding areas (Lebanon, Murfreesboro, Antioch, Hermitage, Mt. Juliet areas)

Building Greater Communities, started business January 1, 1997, and is a private, non-profit corporation governed by a Board of Directors. We have provided independent support coordination services primarily to children and adults who have developmental disabilities and who receive funding through the TN Medicaid Waiver program. Individuals receiving this support come to us via the Tennessee Department ofDisability and Aging. BGC, Inc. is committed to ensuring that all Independent Support Coordination services are centered on assisting the individual (s) in identifying, selecting, obtaining, coordinating, and using both paid services and natural supports to enhance the individuals independence, integration in the community, and productivity as specified in the individuals Person Centered Support Plan (Plan of Care).

Job Duties Include but Are Not Limited to:

Completes and documents ongoing assessments of the individual.

Develops, evaluates, and revises the Person-Centered Support Plan (PCSP).

Meeting specific requirements for content and time frames as set forth by DDA policies.

Provides general education about programs provided, including advocating on behalf of

individuals rights and person centeredness.

Ensures the individual, selected family, and/or Legal Conservator understands the individuals rights afforded to the person and documents such understanding no less than annually.

Coordinates, as authorized, with internal and external resources (i.e. TennCare

Coordinators/Nurses Case Managers, Medical personnel, DHS, DCS, Justice System, Dept. of Education),etc.

Proactively addresses service and support needs for individuals reaching the age of 21 including notifying ISC Supervisor of barriers.

Maintains documented dialogue with designated TennCare Coordinator in situations where support needs are available/provided by TennCare.

Completes and documents monthly contacts, such as in person face-to-face contacts/ virtual contacts with the individual in various settings at various times including visits with the individual in the individual home (where waiver services are provided).

Represents individuals with Intellectual Developmental Disabilities, BGC, Inc., and Self.

Attends all individually scheduled or group scheduled meetings/ training as required by BGC, Inc. and/or by DDA.

Adheres to all directions specific to work needs as set forth by ISC Supervisor.

Job Requirements:

5-6 years of DDA field experience and/or Bachelors degree. Prefer someone also who has hands on experience working with adults and/or children with special needs and familiar with DDA field.

Experience with individuals (adults and/or children) who have developmental disabilities.

Drivers license and dependable vehicle for travel.

Able to pass a background check.

Good computer skills and organizational skills.

Ability to learn and navigate internal systems, such as Practical Health Systems, Therap, SharePoint, Relias, and TennCare portal

Preferred knowledge in 1915c Medicaid waiver program and/or Katie Beckett and TN Strong Families Programs

Ability to write and capture needed details thoroughly for required documentation forms.

Ability to manage and maintain a work schedule and meet timeframes and deadlines.
