Position: Administrative Case manager

Location: Langhorne, PA

Description

The Administrative Case Manager (ACM) provides case management to individuals who are in need of assistance in applying for benefits and resources such as but not limited to: Social Security, Medicaid and Affordable Housing. The Administrative Case Manager provides time-limited extensive case management to individuals to assist them in obtaining additional services and supports including medical coverage, mental health treatment and physical health treatment.

Qualifications:

This position requires a minimum of a Bachelor's degree in a human services field. Experience with working with mental health population is preferred.

Duties/Responsibilities:

  • Coordinates face-to-face case management service delivery to consumers

*Assists clients and families in obtaining financial, transportation, respite, medical, psychiatric and other resources.

*Provides information and referral assistance

*Acts as liaison between Penndel Mental Health Center and external agencies

*Develops new resources and monitors emergence of new resources

  • Fulfills record keeping and reporting requirements

*Participate in regularly scheduled supervision with the Director of Street Outreach and Administrative Case Management Services

Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting, sitting on the floor or performing other work requiring low physical exertion, ability to move quickly in a crisis situation, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time.
