Arkansas Valley Hospice, INC. Registered Nurse in La Junta, Colorado
The Hospice Registered Nurse (RN) provides compassionate, patient-centered care to individuals receiving hospice services. The RN works as part of an interdisciplinary team to assess patient needs, develop and implement care plans, and provide education and support to patients and their families during end-of-life care.
Essential Duties and Responsibilities:
- Perform initial and ongoing patient assessments
- Develop, implement, and update individualized care plans
- Administer medications and treatments as ordered by the physician
- Provide pain and symptom management
- Educate patients, families, and caregivers on disease process, medications, and end-of-life care
- Coordinate care with physicians and interdisciplinary team members
- Complete accurate and timely documentation
- Participate in on-call rotation as required
- Provide emotional support and advocacy for patients and families
Qualifications:
- Current Registered Nurse (RN) license in the state
- Hospice or home health experience preferred but not required
- Strong assessment, communication, and organizational skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
Work Environment:
- Travel required to patient homes, nursing homes, and assisted living facilities
- Flexible schedule
- On-call rotation may be required
