MENOMINEE TRIBAL CLINIC Diabetes Nurse Educator in KESHENA, Wisconsin

JOB REQUIREMENTS: GENERAL DESCRIPTION OF DUTIES Under the general supervision of the Clinic Administrator, the purpose of the position is to provide diabetic education to patients and family members primarily within the Clinic, but will also work in collaboration with Community Health Nurses on prevention activities and follow up with home visits when necessary. EXAMPLES OF ESSENTIAL FUNCTIONS Gather data and assist with the data management systems development for tracking patients and identify those who will require intensive education efforts. Assess patient charts prior to patients being seen in the Clinic. Maintain records of nursing care and other treatments given to patient, collect data on patient behavior and treatment related items. Monitor patients through home visitation to determine patient needs and compliance level if needed Prepare individual treatment plans as directed by physician, instructing the individual and or family of the plan. Perform diabetic chart audits with assistance from Diabetes Program staff. Participate in community education, screening activities and committees that promote health and address health concerns. Execute and Maintain the Diabetes Program Budget and bill paying Coordinate the Diabetes Program and oversee all aspects of the Diabetes Program grant activities Prepare the yearly grant application and submit any grant reports as the grant suggests on progress made on the grants goals and objectives. Create (or prepare) reports (PI, Annual) on the diabetes program progress made on activities and goals. Attend conferences and seminars to earn CME\'s to maintain license or certifications needed for the position. Assist the wellness program with their program activities and cover their department when staff are not available or out of the office. Perform related duties as directed. ***** OTHER EXPERIENCE AND QUALIFICATIONS: MINIMUM TRAINING AND EXPERIENCE Current Wisconsin RN Licensure.. Bachelor\'s of Science in Nursing (BSN) preferred. with training emphasis in Diabetes education; supplemented by minimum three (3) years previous experience and/or training that includes experience in Diabetes education, basic health maintenance and simple nursing procedures; or an equivalent combination of training, and experience. SPECIAL REQUIREMENTS Candidates shall be subject to a Caregiver background check. Candidates selected for hire will be subject to a drug test and employment will be contingent on results of said test. Must possess a license to practice Professional Nursing in Wisconsin. Preferably possess a Diabetes Care & Education Specialist (CDCES) Certification or be eligible to sit for CDCES exam within 1 year of hire.. Ability to display sensitivity and tact in interpersonal relations.
