Job Summary
The Chemical Dependency Counselor provides direct services to Behavioral Health clients identified substance use disorders. The Chemical Dependency Counselor may also assist with Henu' Community Wellness Court clients identified with co-occurring mental health and substance use disorders. The Chemical Dependency Counselor is responsible for the assessment, diagnosis and treatment of Substance Use Disorder (SUD) clients, development and ongoing review of treatment plans, transition planning, and individual and group counseling in a supervised setting, making appropriate mental health referrals when necessary. The CDC is expected to establish and maintain a supportive and therapeutic environment. It is expected that they will participate in supervision with chemical dependency manager and or other members of the behavioral health team in pursuit of higher levels of certification. This position is responsible to maintain client files in accordance with applicable policies and regulations.
Essential Functions
- Provides substance use assessments of clients to determine appropriate treatment level
- Develops or participates in the development of goals, objectives, and interventions for client treatment plans
- Conducts period reviews of levels of care for clients and determines need for transition/discharge readiness
- Provides individual and group counseling to assigned clients/family members
- Facilitate groups regarding substance use, addiction, and recovery
- Coordinates services with Behavioral Health and Henu' Court teams for a multidisciplinary approach
- Actively participates in weekly clinical and individual supervision meeting with Chemical Dependency Manager and other Chemical Dependency Counselor(s), including peer consultation and peer record review
- Coordinates the activities of and supervises Peer Support Mentors in their work with Henu' clients
- Maintain client records in accordance with Tribal and Medicaid standards
- Follow all grant requirements and reporting grant requirements to supervisor
