Dartmouth Health Registered Nurse, RN. Prenatal Care Coordinator in Keene, New Hampshire

Responsible for screening and assessing pregnant patients to provide support in accessing appropriate prenatal care. This includes assessing for risk and implementing risk reduction interventions and proper care coordination. Identify antenatal patients that are eligible for various services and coordinate appropriate referrals/appointments, collaborating with the physician Chair and Lead APPs as needed for risk stratification. Create and maintain a tracking system to monitor patients throughout their pregnancy and postpartum follow-up. Monitor patient appointments, and follow up with patients that miss appointments. Provide education and training of finger sticks and/or insulin injections, home blood pressure monitoring, and urine dips, etc. Schedule all antenatal testing and coordinate consult visits with specialists and providers. Perform chart reviews at identified intervals for risk assessment and identify best practice gaps. All other duties as assigned.

Graduate of an accredited nursing program. BSN preferred. 3-5 years of OB/GYN, L&D, or care coordination experience preferred. BLS within 3 months of hire and maintained in accordance with quarterly RQI requirements. Completion of AWHONN Introduction to Fetal Heart Monitoring course within 6 months of hire.
