Job Description
The Nurse Care Manager (NCM) is responsible for lateral integration of care across all settings in which health care is delivered to his/her assigned patients. Through the assimilation and application of evidence-based information, the NCM designs, implements, and evaluates client treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working with the health care system to promote an environment that will empower his/her assigned patients to manage their illness(es) and achieve optimal outcomes. In addition, the NCM will provide and manage care, and assist in the tracking of healthcare outcomes for his/her Health Home enrollees.
Minimum Requirements
BSN or the equivalent in a combination of an ADN and relevant experience
Current Missouri License as a Registered Nurse
Minimum of three (3) years experience working in a healthcare setting
Excellent written and verbal communication skills
Must be able to read and speak the English language
Proficient in Word, Excel and Power Point
Current BCLS
If driving is a requirement for the employee, he/she must meet the requirements specified in the Fleet Driving Safety and Compliance Policy and must continue to meet these requirements as a condition of employment
Preferred Requirements:
Community service experience
Multi-language skills
