Licensed Practical Nurse (LPN) MDS Coordinator Responsibilities:
• Coordinate and manage the Minimum Data Set (MDS) process, including collecting and documenting relevant information
• Ensure accurate and timely completion of MDS assessments, care plans, and progress notes
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Provide education and support to patients, families, and caregivers on care plans and services
• Maintain accurate and detailed records of patient assessments, care plans, and progress
• Participate in quality improvement initiatives to ensure high-quality care and patient satisfaction
• Develop and maintain strong relationships with patients, families, and healthcare professionals
Licensed Practical Nurse (LPN) MDS Coordinator Requirements:
• 1 year of experience as an MDS Coordinator or in a related field
• High school diploma or equivalent required
• Strong communication, organizational, and problem-solving skills
• Ability to work in a fast-paced environment and prioritize tasks effectively
• Passion for delivering exceptional patient care and customer service
