Fedcap Transition Specialist in Jamaica, New York

Position Summary:

The Transition Specialist is responsible for comprehensive, culturally competent services to individuals detained in the city jails and upon reentry to IMPACT.

What You'll Do:

Case Management:

  • Conduct thorough assessments and, through a motivational interviewing process, develop individualized person-centered Comprehensive Service plans for 100% of participants on caseload.

  • Provide crisis intervention and engage in safety planning.

  • Provide individualized services to 100% of Participants on caseload from the point of orientation to exit.

  • Provide evidence-based clinical support as needed.

  • Facilitate care from Rikers through successful re-entry in the community. This includes ensuring that connections are made to appropriate internal and external services as required to meet the needs of the participants.

  • Work in conjunction with Reentry Mentors and Reentry Coordinator to ensure seamless transition and continuity of caregiver for 100% of participants on caseload.

    Transition Planning:

  • Conduct quarterly presentations of reentry services offered in the jail.

  • Conduct thorough assessments and, through a motivational interviewing process, develop individualized person-centered transition plans for 90% the assigned houses.

  • Expedite communication and/or case conferences with DOC, CHS, jail-based providers, defense teams, community partners, and positive supports.

    Performance Management and Reporting:

  • Maintain regular contact with participants to meet program standards and achieve program retention for 100% of the participants on caseload.

  • Sustain open communication with program participants through face-to-face contacts, phone conversations, emails, and text messages.

  • Provide outcomes reporting every month.

  • Maintain 100% HIPAA and Informed Consent Compliant Documentation

  • Manage participant files, including progress notes and reports on progress toward goals.

  • Provide weekly reports to the team and Program Director.

  • Participate in regularly scheduled supervision and ongoing training.

  • Attend meetings and participate in professional development training offered.

  • Enter case notes for every job seeker in FedcapCARES daily.

    You're a Great Fit For this Role If:

  • You have a Bachelor’s degree in human services, psychology or special education, or a related field from an accredited college or university.

  • Bring a minimum of 4 years working with the population, plus a HS diploma or equivalency accepted instead of a bachelor’s degree.

  • Experience working with formerly incarcerated individuals preferred.

  • Case Management experience preferred.

  • Must be able to receive entrance clearance to Rikers Island.

  • Bilingual (English/Spanish) preferred.

    Compensation:

  • $66,300 annually
