Position Summary
[The Intake Coordinator supports the admissions and assessment process at TPC Wellness by assisting with client intakes, gathering required documentation, completing screening tools within scope of certification, and coordinating information between clients and the clinical team. This position works under the supervision of licensed clinical staff and plays a key role in ensuring accurate, timely, and compliant intake processes. Essential Duties and Responsibilities
[Report directly to Operations Director]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Assists Community Outreach Director with scheduling transport for TPC individuals served.]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Assist clients with the intake and admission process in a respectful and supportive manner]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Review, explain, and assist with completion of informed consent documentation under supervision]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Obtain and process releases of information in compliance with HIPAA and 42 CFR Part 2]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Collect intake information and complete screening tools within scope of CDCA certification]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Assist licensed staff with gathering information for bio-psycho-social assessments]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Demonstrate basic knowledge of Substance Use Disorder (SUD) symptoms and treatment services]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
[Document client information accurately and timely in the electronic health record]{times="" new="" roman",="" times,="" serif;="" font-size:="" 10pt"=""}
