JOB REQUIREMENTS: Requisition: 2026-182023 Pharmacist / PRN Job Locations US-WI-HUDSON ID 2026-182023 Line of Business OnePoint Patient Care Position Type PRN Our Company OnePoint Patient Care Overview We are looking for a Pharmacist to join our team of providing the highest quality of care, reliability, and support to our patients, caregivers, and families. In this role, the Pharmacist will be responsible for dispensing medications, providing clinical advice, as well as delivery services. Shift: On call Sunday-Wednesday night and possible Saturday/ Sunday 12:00pm-4:00pm rotation Responsibilities Provides clinical consultations to patients as well as recommendations to nurses, physicians, and other hospice personnel Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes Answers customer questions and resolves customer issues in a timely manner Retrieves data in the pharmacy system Manages medication orders Ensures proper storage and security conditions for medications Maintains a productive, orderly, and error-free work environment Qualifications Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree Current and good standing Pharmacist License Strong communication skills, customer service, and problem-solving skills About our Line of Business OnePoint Patient Care, an affiliate of PharMerica, is the nation\'s leading independent provider of community-based hospice pharmacy and pharmacy benefits management (PBM) services - offering hospices nationwide flexible and adaptable solutions for their hospice pharmacy needs.
