WORKFORCE RESOURCE INC Case Manager Transitional Jobs in HUDSON, Wisconsin
JOB REQUIREMENTS: The Case Manager is responsible for the recruitment and retention of participants into the Transitional Jobs (TJ) program serving St. Croix, Pierce and Pepin counties, providing individualized case management and supportive services; development and implementation of individualized employment and training plans; establishment of SMART goals; coordination with partner agencies of needed services such as education, training, formal assessments, etc.; development of participant work sites and job coaching, and on-going follow-up and support. The Case Manager has the ultimate responsibility for the case and is responsible to meet individual performance goals. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Skills required include ability to assess vocational skills and needs of defined service populations; knowledge of community resources and potential participants, positive work ethic, knowledge of business needs and the economy in the geographical area assigned; effective interpersonal and counseling skills; group facilitation and organizational skills; ability to keep accurate records/case noting and data entry; ability to work independently and travel within the region assigned. ***** APPLICATION INSTRUCTIONS: E-Mail a Rsum: talfordk@workforceresource.org
