Registered Nurse - Hornell IP Clinic <br> FT (24 hours per week) <br> Schedule: Two 12 hour shifts per week, with rotating weekends <br> Hours: 7:00pm to 7:00am <br> Location: 7309 Seneca Rd. North Hornell , NY 14843 <p>

The Registered Nurse (RN), Integrated Care, provides compassionate, patient-centered care in a behavioral health setting that integrates substance use disorder (SUD), mental health (MH), and opioid treatment program (OTP/Methadone) services. The RN is accountable for ongoing assessment, coordination, and delivery of medical care while working closely with the multidisciplinary team. All care is provided in accordance with NYS OASAS, NYS OMH, and SAMHSA regulations, agency policy, and established standards of nursing practice. <p>

Essential Job Functions <ul> <li>Maintain a cordial, respectful, and welcoming presence with all clients at all times. Supporting client comfort, safety, dignity, privacy, and confidentiality throughout all interactions. <li>Conduct comprehensive assessments including social, medical, psychiatric, and substance use history in collaboration with providers. <li>Administer medications including Methadone and other MAT (Medication-Assisted Treatment) under protocol and monitor patients for adherence and side effects. <li>Perform technical services such as EKGs, phlebotomy, collect vital signs, urine drug screening, and breathalyzers. <li>Provide trauma-informed emergency care and interventions needed in crisis or life-threatening situations. <li>Support daily operations including dosing, patient check-ins, and clinic workflow. <li>Assist in medication ordering, deliveries, and prior authorization processes. <li>Participate in scheduling support, appointment prep (e.g., I-STOP/PMP checks, labs), and follow-up planning. <li>Manage specimens, test reports, and collaborate with external laboratories and providers as needed. <li>Ensure the proper maintenance, storage, and security of controlled substances including Methadone, following all state/federal protocols. <li>Review and interpret outside medical exams and labs, integrating into treatment planning. <li>Offer follow-up instructions and support post-visits to enhance treatment engagement and health outcomes. <li>Provide culturally competent, whole-person education regarding diagnoses, treatment options, medication use, and recovery support. <li>Communicate client needs and changes in condition to the relevant team members in a timely and professional manner. <li>Support medical intervention programs and treatment plans that reflect a whole-person, trauma-informed, and recovery-focused approach. <li>Ensure the proper maintenance, storage, and security of controlled substances including Methadone, following all state/federal protocols. <li>Ensure compliance with OASAS, OMH, SAMHSA, NYS Public Health Law, and 42 CFR confidentiality requirements. <li>Function as a member of an interdisciplinary care team, supporting care planning, coordination, and communication across SUD, MH, and OTP/MAT services. <li>Document patient encounters, medication administration, assessments, and care plans accurately and in a timely manner. <li>Serve as a resource for staff and patients regarding SUD/MH diagnoses, MAT, and integrated medical care. </ul>

Qualifications: <ul> <li>Associates Degree in Nursing (Bachelors preferred). <li>Current New York State Registered Nurse license in good standing. <li>Valid driver's license and reliable transportation. <li>Minimum one year of experience in behavioral health, SUD, or a related clinical setting. <li>Must pass criminal background and other pre-employment clearances. </ul>
