The Queen's Health Systems Patient Care Coordinator (RN Required) - Transplant Center (Full-Time, 40, Day) in Honolulu, Hawaii

RESPONSIBILITIES

Position Summary

The Patient Care Coordinator (RN) is a key member of the multidisciplinary transplant team responsible for coordinating complex patient care across the transplant continuum, including evaluation, waitlisting, transplantation, and long‑term follow‑up.

This is a non‑bedside, coordination‑focused nursing role well suited for nurses with strong clinical judgment, chart review skills, computer proficiency, and patient relations experience.

Work Schedule & On‑Call Expectations

  • This is a weekday, daytime role (Monday–Friday, generally standard business hours).

  • The position includes participation in a shared on‑call rotation , with coverage distributed among team members and supported by established processes.

Key Responsibilities

  • Coordinate care for transplant patients across all phases of the transplant process

  • Serve as a clinical liaison between patients, families, physicians, and the interdisciplinary team

  • Perform comprehensive clinical assessments and detailed chart reviews

  • Ensure compliance with UNOS, CMS, and regulatory requirements

  • Provide patient and caregiver education and ongoing clinical support

  • Monitor diagnostic testing, laboratory results, and follow‑up care

  • Participate in shared on‑call coverage to support transplant patients as needed

Minimum Qualifications

Education, Certification & Licensure

  • Bachelor’s degree in Nursing

  • Current Hawaiʻi licensure as a Registered Nurse

  • Current BLS certification for healthcare providers

  • Current certification from the American Board for Transplant Certification as a Certified Clinical Transplant Coordinator (CCTC) , or certification must be obtained within two (2) years of entrance into the position

Experience

  • Two (2) years of clinical experience as a Registered Nurse

  • Previous transplant experience is not required for this role

  • The Queen’s Transplant Center provides comprehensive onboarding and specialty training. Registered Nurses with strong medical‑surgical backgrounds, experience in care coordination, chart review, patient education, and outpatient or clinic‑based settings have been highly successful as Patient Care Coordinators.

  • Experience in computer applications, such as Microsoft Word and Microsoft Excel
