Registered Nurse Full-time - M-F 8:00 am- 4:00 pm \$37.00 - \$40.00 / hr. DOE Job Description: Performs assessments, admission, 2week, 4week and 60 days visits w/documentation completed within 2 days. Conduct post-hospital, creates an individual service plan/meds sheet. Assist in delivery of program services as needed.
