The Essentials
Pay: $36.07
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Travel: 50% – 75% local travel (Hialeah area) with mileage reimbursement.
Location: Hybrid/Field-based. Candidates must reside in one of the following ZIP codes: 33010, 33012, 33013, 33014, 33015, 33016, 33018
What You’ll Do
As a Case Management Coordinator, you are the bridge between members and the care they deserve. Your day-to-day includes:
Holistic Assessments: Conducting both telephonic and in-person assessments in member homes, assisted living facilities, and nursing homes.
Care Planning: Developing and managing individualized care plans for a diverse caseload, including medically complex members.
Clinical Coordination: Partnering with PCPs, specialists, and community resources to streamline care.
Case Oversight: Handling prior authorizations, medication reviews, and condition management education.
Advocacy: Proactively identifying and resolving barriers to care to improve long-term health outcomes.
Key Requirements (Non-Negotiable)
To be considered, candidates must meet the following criteria:
Bilingual Mastery: Full fluency in English and Spanish (written and verbal).
Hyper-Local Residency: Must live in one of the Hialeah ZIP codes listed above.
Mobile Readiness: Ability to travel up to 75% for in-person member visits.
Tech-Savvy: Comfortable navigating digital health records and remote communication tools.
Self-Driven: High level of organization and the ability to work independently in the field.

Pay Details: $36.07 per hour
