Certified Nurse Assistant (CNA)/Home Health Aide

Location: Meriden Region

Schedule: Mon - Fri, 8 am - 5 pm, one weekend every 4 weeks

Status: Full Time

Sign on Bonus $5,000

Trinity Health Of New England At Home is national home care, palliative care, and hospice organization. We provide comprehensive home care and end‑of‑life services in the comfort of patients’ homes.

We care for patients and their families through skilled nursing, therapy services, medical social work, hospice care, and bereavement support.

Position Purpose:

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

Must be a Certified Nurse Assistant in the state of Connecticut.

Position Details:

Schedule also includes two rotating holidays per year.

Sign on Bonus: $5,000 paid out over two years.

This position is 100% home care assignments working with hospice patients.

What you will do:

  • Follows the RN‑developed care plan and reports any client condition changes.

  • Assists with personal care: bathing, grooming, toileting, transferring, walking, and dressing.

  • Supports household needs: meal prep, feeding, light cleaning, bed‑making, and laundry.

  • Takes and records vital signs as ordered.

  • Performs simple therapy‑assigned exercises (e.g., range of motion).

  • Helps clients self‑administer medication as assigned by an RN; does not administer medication.

Minimum Qualifications:

  • High School diploma or GED required.

  • Must be a Certified Nurse Assistant in the state of Connecticut.

  • 1 year of experience as an aide in an institution, home care, or hospice setting.

  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.

  • Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis.

  • Must have current Driver’s license and reliable transportation to and from work site.
