Monarch NC, a leader in trauma-informed, high-quality services for individuals with intellectual and developmental disabilities, mental illness, and substance use disorders, is seeking a compassionateLicensed Practical Nurse (LPN)to join our Assertive Community Treatment (ACT) team inGreensboro, North Carolina.

What You'll Do

As anLPN on our ACT team, you will play a key role in providing community-based support that helps adults maintain stability, build skills, and participate fully in daily life. In this dynamic, community-focused role, you will:

  • Provide nursing services, including medication administration, monitoring, assessments, and interventions.
  • Support individuals in developing daily living skills, personal routines, and healthy habits.
  • Help people access community services, resources, and support networks.
  • Conduct daily needs assessments and adjust support strategies as needed.
  • Collaborate closely with ACT team members, inpatient treatment programs, detox facilities, residential programs, and community resources.
  • Contribute to person-centered planning that promotes independence, choice, and long-term success.

What We're Looking For

To qualify, candidates must have:

  • AHigh School Diploma or equivalent.
  • A currentNorth Carolina Licensed Practical Nurse (LPN)license.
  • At least 1 year of experienceworking with adults with a mental health diagnosis and/or substance use disorder.
