Heritage Ministries is in growth mode and currently seeking a Clinical Quality and Education Nurse to join our team! The Clinical Quality and Education Nurse provides clinical leadership and support to skilled nursing facilities through education, quality improvement initiatives, documentation review, and regulatory readiness activities. The role works collaboratively with facility Administrators and Directors of Nursing to strengthen clinical systems, improve documentation practices, and support regulatory compliance. The position focuses on education, clinical quality audits, documentation review, and survey preparedness to help facilities maintain high standards of resident care and regulatory compliance . Responsibilities:

  • Oversee Clinical education, including the CNA program

  • Provide education and coaching to nursing staff and leadership.

  • Support competency development for nurses and nurse managers

  • Provide education related to clinical documentation standards.

  • Assist staff in understanding regulatory expectations and survey requirements

  • Provide training related to PDPM documentation and clinical care documentation.

  • Support development of leadership skills among Directors of Nursing and nursing supervisors.

  • Partner with facilities on employee physicals, vaccine, and other regulatory requirements

  • Conduct clinical chart reviews to assess quality of care and documentation practices.

  • Conduct chart reviews to ensure documentation supports medical necessity of services and payer review requirements.

  • Collaborate with facility MDS Coordinators to ensure accurate completion of MDS assessments.

    Qualifications:

  • Active New York State Registered Nurse license

  • Minimum 5–7 years of nursing leadership experience in skilled nursing or long-term care

  • Strong knowledge of CMS regulations and New York State Department of Health requirements

  • Bachelor of Science in Nursing (BSN) - preferred

  • Experience with MDS and PDPM documentation - preferred
