? How You’ll Make an Impact as a Speech-Language Pathologist
As a crucial member of our home‑based care team, you will:
Champion Elara Caring’s mission and philosophy
Assess and evaluate patient needs, establishing a personalized Plan of Care
Perform initial OASIS assessments and ongoing re-evaluations
Ensure patient safety through thoughtful care planning and resource coordination
Collaborate with physicians, supervisors, and interdisciplinary team members
Implement and revise Plans of Care based on patient progress
Provide direct speech-language pathology services according to clinical standards
Select therapeutic materials tailored to patient needs
Educate and support patients and caregivers on home therapy techniques
Maintain accurate, timely documentation and communication
Work in a dynamic, physically demanding environment with flexibility for irregular hours
✅ What You’ll Need
Master’s degree from an accredited program in Speech-Language Pathology
Current, unrestricted SLP license in Indiana
Minimum 1 year of clinical SLP experience
Ability to travel 50% within your service area
Reliable vehicle, valid driver’s license, and auto insurance
Ability to lift 50–100 lbs. and assist with patient transfers
