County of McLeod Social Worker - Disability Waiver Case Manager - Rule 185 DD in Glencoe, Minnesota
Job Duties
The Social Worker – Disability Waiver Case Manager is responsible for providing comprehensive case management services to individuals with disabilities, ensuring they have access to needed social, medical, educational, and other supportive resources. This role involves coordinating and evaluating services, developing and reviewing MnCHOICES Support Plans, and performing essential administrative case management tasks. The Case Manager ensures that all service planning and delivery follow a person-centered approach and monitors service quality, provider compliance, and adherence to program standards. Additional responsibilities include providing Rule 185 Developmental Disabilities Case Management services, facilitating communication among clients, families, and service providers, and maintaining accurate documentation while complying with data privacy requirements. The position also supports overall agency operations through related duties as assigned.
Minimum Education Required
The position requires a bachelor’s degree from an accredited four‑year college or university, with a major in social work, psychology, sociology, or a closely related field. Candidates may also qualify with a bachelor’s degree in any field if they have at least one year of experience as a social worker in a public or private social services agency. In addition, individuals licensed as a public health nurse or registered nurse are eligible to meet the educational requirement for this role.
Minimum Experience Required
Experience for this position can be met in several ways. Candidates with a bachelor’s degree in any field must have at least one year of experience working as a social worker in a public or private social services agency. Individuals who hold a bachelor’s degree in social work, psychology, sociology, or a closely related field are not required to have prior experience, though it is beneficial. Public health nurses and registered nurses also meet the experience requirement based on their professional background. Additional preferred experience includes holding Social Worker Licensure or qualifying as a Qualified Developmental Disability Professional (QDDP).
