County of McLeod Social Worker - Disability Waiver Case Manager - CCB in Glencoe, Minnesota

Job Duties

The Social Worker – Disability Waiver Case Manager is responsible for delivering comprehensive case management services to individuals receiving disability waiver support, ensuring they have access to necessary social, medical, educational, and community-based services. This position coordinates and evaluates services, develops and reviews MnCHOICES Support Plans, and completes essential administrative case management tasks. The role includes providing CCB Case Management services, facilitating person-centered planning, and monitoring the quality and effectiveness of service delivery across providers. The Case Manager supports individuals in navigating waiver and State Plan services and ensures compliance with program requirements and quality assurance standards. Additional responsibilities include maintaining accurate documentation, collaborating with families and multidisciplinary professionals, and performing other related duties to support the overall operations of the agency.

Minimum Education Required

The position requires a bachelor’s degree from an accredited four‑year college or university, with a major in social work, psychology, sociology, or a closely related field. Applicants may also qualify with a bachelor’s degree in any discipline if they have at least one year of experience working as a social worker in a public or private social services agency. In addition, individuals licensed as a public health nurse or registered nurse also meet the educational requirement for this role.

Minimum Experience Required

Experience requirements for this position can be met in several ways. Candidates with a bachelor’s degree in any field must have at least one year of experience working as a social worker in a public or private social services agency. Applicants who hold a bachelor’s degree in social work, psychology, sociology, or a closely related field are not required to have prior experience, although such experience is beneficial. Individuals who are licensed as a public health nurse or registered nurse also meet the experience criteria based on their professional background. Additionally, while not required, holding Social Worker Licensure is preferred and may strengthen a candidate’s qualifications.
