GENERAL DESCRIPTION:
Responsible for the direct supervision of assigned Direct Support Professionals (DSPs). Hires, supervises, evaluates and provides ongoing training to designated staff and volunteers.
ESSENTIAL FUNCTIONS:
Regulatory and Corporate Compliance
Ensures compliance with regulations and agency policies and procedures.
Ensures required documentation is completed in an accurate, legible and timely manner.
Ensures the agency's incident management process is followed.
Provides input in the development and revision of policies, procedures and agency forms as needed.
Ensures Chain of Command is followed when necessary and instructions are followed as stated in policies and procedures.
Ensures that the DSP Core Competencies are integrated into the culture and day to day operations in accordance with Mozaic policy. This includes coaching, counseling, clinicals, Progressive Guidance, DSP evaluations after 6 months of employment for new hires and then on an annual basis thereafter.
Financial Management
Ensures adherence to the Program budget.
Ensures accurate management of individual's funds.
Ensures all individual and program financial documentation is completed according to policies and procedures.
Person Centered Services
Ensures service provision to individuals as planned in their Life Plan, which includes the person's Individual Protective Oversight Plan (IPOP), Staff Action Plan (SAP), Support Plans, and/or other safeguards while ensuring all related documentation is completed in a contemporaneous manner.
Ensures all activities are implemented as outlined in the SAP Plan and, as applicable, the Life Plan/IPOP.
Plans and coordinates daily activities.
Acts as an advocate for individuals.
Ensures individuals are assisted and supported with community integration.
Respects the individual's right to privacy and confidentiality through appropriate use of written and verbal communication.
Works collaboratively with others to ensure the individual's health and wellbeing.
Ensures emergency medical procedures are followed and consults with the medical chain of command when necessary.
Ensures adherence to all Plan of Nursing Services (PONS).
Reports and documents all individual medical concerns to the appropriate personnel.
Maintains regular communication with all members of the individual's team.
Program/Service Management
Ensures all assigned staff are fully trained per agency requirements to implement the DSP duties and responsibilities.
Maintains an environment which encourages and facilitates optimal personal development.
Ensures completion of assigned daily responsibilities.
Completes specific duties as assigned.
Ensures completion of documentation, filing and general office maintenance and other routine operational reports and any other paperwork as assigned.
Assists in the identification and development of new services and support opportunities that provide quality of life choices.
Physical Plant and Safety
Ensures the site is a safe, clean environment and immediately addresses areas of concern.
Maintains knowledge of fire evacuation plan and ensures plan is followed, makes changes as necessary.
Ensures all DSPs practice standard precautions and other appropriate infection control practices.
Ensures site vehicles are maintained and ensures completion of vehicle inspections.
Contacts with Others
Develops and maintains positive relationships both internally and externally with families, local businesses, service providers, etc.
Maintains effective working relationships with service providers, community resources and families.
Ensures that internal and external written and verbal communications regarding the agency are positive and reflect the agency's Mission, Vision and Values while respecting the individuals' rights to privacy and confidentiality.
Ensures all concerns are addressed in a timely and professional manner.
ADDITIONAL FUNCTIONS:
Participates in agency committees, initiatives, projects and trainings, as assigned.
Assists with coverage in the agency as assigned.
Maintains certifications and annual training updates, as required.
Use of private vehicle may be required to transport the individuals supported.
Performs other duties as assigned.

Strong written and verbal communication skills.
Able to operate office equipment, computers and medical equipment.
Possesses a valid NYS driver's license that is acceptable to the agency's insurance carrier and maintains a safe driving record per agency policy. Is able to operate wheelchair vans.
- Maintains a working telephone and ensures agency has updated information.
Medication Administration Certification, as required.
EDUCATION AND EXPERIENCE:
Minimally three (3) years of experience working with people with developmental disabilities.
High School diploma or equivalent required.
Two (2) years of supervisory or leadership experience requires.
Minimum qualifications can only be waived by the Executive Vice President of Administration.
