Licensed Practical Nurse LPN Weekend Private Duty | 1:1 Patient Care

As a Private Duty Licensed Practical Nurse , you deliver focused, one-on-one care in the comfort of the patient’s home. You are not managing a hallway of patients. You are providing personalized care where it matters most.

WHAT YOU’LL DO

• Provide direct private duty nursing care in the home

• Review Plan of Care and coordination notes before visits

• Use clinical judgment to assess and monitor patient conditions

• Report significant findings to RN Case Manager or Clinical Team Manager

• Document care accurately and submit notes timely

• Collaborate with physicians and care teams

• Adapt scheduling to meet client needs

WHAT YOU NEED

• Graduate of an accredited school of nursing

• Active LPN license (state or Nurse Licensure Compact)

• Minimum 1 year of clinical nursing experience

• Reliable vehicle, valid driver’s license, and current auto insurance

• Ability to sit, stand, bend, lift, and move intermittently

• Ability to lift 50–100 lbs as needed
