Adecco US, Inc. Physical Therapist in Fort Morgan, Colorado
Adecco Healthcare & Life Science is working with our client to hire a Home Health Physical Therapist (PT) in Fort Morgan, CO.
This is an in-person/on-site role providing care in patients’ homes within the surrounding service area.
Type: Contract (04/20/2026 – 07/19/2026) – possibility of extension
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (No Call)
Pay Rate: $48.00–$50.00/hour, based on experience
Primary Responsibilities Include:
• Perform comprehensive patient evaluations including mobility, strength, balance, gait, pain, endurance, and functional limitations
• Assess home environments for safety risks and fall prevention opportunities
• Develop individualized plans of care with measurable goals, visit frequency, and duration
• Complete all required home health documentation, including OASIS and physician orders
• Deliver skilled physical therapy interventions such as therapeutic exercise, gait and balance training, transfer training, and stair navigation
• Train patients on safe use of assistive devices (walkers, canes, wheelchairs)
• Provide functional training in real-life home settings (bed mobility, bathroom safety, household ambulation)
• Modify treatment plans based on patient response and medical complexity
• Educate patients and caregivers on home exercise programs, fall prevention, and energy conservation techniques
• Train caregivers on safe mobility and transfer techniques
• Coordinate care with physicians and interdisciplinary team members (RN, OT, SLP)
• Plan and support safe discharge, including maintenance programs and referrals as needed
Required Qualifications:
• Master’s (MPT) or Doctorate (DPT) in Physical Therapy
• Active Physical Therapist license (Colorado or compact state, if applicable)
• Home Health experience required
• OASIS documentation experience required
• Ability to travel daily within a 50-mile radius
• Valid driver’s license and reliable transportation
• Experience with Homecare Homebase (HCHB) - preferred

Pay Details: $48.00 to $50.00 per hour
