Position Details

Shift: Evenings
Hours: 2:00 PM to 10:30 PM
Required Certification: Wisconsin State-issued CNA Certification
Job Summary:
Provides direct personal care to residents according to an established care plan and in compliance with state and federal regulations. Works closely with the team to ensure that daily assignments are completed including ADL (activities of daily living) flow sheets, daily rounds, meal percentages, intake & outputs (I & O's), vital signs, calorie counts, weights, and BM's. Attends care conferences on primary residents whenever possible. Alerts team nurse when care cards need to be updated. Leads by example (always follows dress code, has gait belt, badge, and hand gel). Floats throughout the facility as needed for equal distribution of lead CNAs on all neighborhoods. Promotes the safety of residents and staff.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
Supports orientations of new and floating organizational staff and agency staff and provides training. Provides feedback to peers and team members regarding care delivery. Supports facility and nursing department standards of practice and policies. Seeks opportunities to be cross trained in specific skills. Floats to other neighborhoods or shifts as needed. May offer input into CNA interviews/evaluations. Attends monthly meetings, and reads all minutes from meetings when unable to attend. Supports CNA efforts to attend their monthly meetings and other shared accountability team meetings by assisting with and reassigning tasks during their absence.
Provides quality care and services for residents in a dignified and respectful manner that meets individual resident care needs in accordance with the resident's plan of care. Provides care that helps to maintain the highest level of resident independence. Monitors and reports changes of condition. Completes required records to document resident care. Assists nurses with vital signs, weights, and specimen collection.
Provides care to prevent skin breakdown such as utilizing specialty mattresses, foam boots, Cavilon to heals, turning/repositioning schedules, toileting schedules, cushions in all wheelchairs. Promptly reports rashes, open areas, edema, blisters, skin tears, or bleeding to nurse.
Responds promptly to fall alert alarms. Ensures that restraint alternatives are in place and are activated as indicated including personal alarms, cushion alarms, wheelchair belts, tumble mats, alarm mats, bolstered beds, hip protectors. Remains alert to fall risks for residents and reports concerns to nurse.
Ensures quality end of life care. Uses comfort basket to report resident pain/restlessness to nurse. Demonstrates compliance with post-mortem care procedures. Participates in departure ceremony.
Complies with safety policies/procedures to ensure safe practices for residents, staff, self, and others. Maintains compliance with gait belt use for all manually assisted transfers, locomotion/transfer/lifting devices, resident positioning, and always uses a mechanical lift to lift residents from the floor. Uses good body mechanics while on duty. Identifies wandering residents and responds to door alarms. Reports safety incidents, concerns, or hazards to the supervisor or other responsible parties as indicated.
Complies with infection control standards during tours of duty including hand hygiene, appropriate use of personal protective equipment, and isolation precautions. Understands to use handwashing with soap and water when dealing with gastrointestinal illnesses. Helps to maintain residents in isolation when indicated. Participates in mandatory vaccination programs. Does not work when contagious.
Completes whiteboards within the first hour of duty on AM/PM shift. Answers call lights as soon as possible to meet resident needs in a timely manner. Avoids "toxic" pleases while on duty. Takes action to correct customer service issues immediately.
Completes all assigned documentation requirements including guidelines for resident care, BM outputs, meal intakes, restorative care, behaviors, and others. Keeps access code for computer confidential at all times.
Maintains privacy and confidentiality for residents at all times including resident information in any format such as paper, electronic, video, photographic, and conversation.
Works in a constant state of alertness and safe manner.
Performs other duties as assigned.
EDUCATION
- Completion of an approved nurse aide training program and passing grade on state of work location required exam.

PHYSICAL REQUIREMENTS
Constant standing and walking.
Frequent use of hearing and speech to share information through oral communication.
Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent reaching, gripping, and keyboard use/data entry.
Frequent use of vision for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Occasional bending, stooping, crawling, kneeling, sitting, squatting and repetitive foot/leg and hand/arm movements.
Occasional use of vision to judge distances and spatial relationships.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Occasional lifting/moving patients.
Occasional use of smell to detect/recognize odors.
Occasional driving.
